WHAT ARE THE STRUCTURES OF THE SHOULDER JOINT?

The shoulder joint comprises of bony structures that are surrounded by muscles,tendons and ligaments.Tendons attach muscles to the bones so that joint motion is possible.Ligaments stabilize the three bones of the shoulder joint.

WHAT ARE THE CAUSES OF SHOULDER PROBLEMS?

Most shoulder problems are caused by soft tissue injuries.Forced activities,poor Office ergonomy or hard sports may cause injuries.Shoulder pain can be local or radiate to the arm.Sometimes pain can be misleading and may originate far from the shoulder joint.The causes of this reflecting pain may be; liver and gall bladder,cardiac diseases,cervical problems.

Shoulder problems can be seen in every individual,no matter what the age,race,gender,ethnicity is.

Rotator Cuff Injuries (Tendinosis and Bursitis)
Most frequent tendon problems of the shoulder joint is impingement of the tendons between the structures.
Bursitis is the inflammation of the bursa (pouches with fluid inside to protect the joint).Sometimes it occurs in disorders like Rheumatoid Arthritis,forced activities,ball pitchers over the head can cause injury and impairment of the shoulder.
Rotator Cuff tears are due to overuse by ageing.They can be painless or painfull according to its localisation.

Rotator Cuff Injuries (Tendinosis and Bursitis)

TREATMENT

Immobilisation of tha joint,
Cold pack ,
Non-Steroidal Anti-inflammatory drugs (NSAIDs),
Physical Therapy modalities (magnetic field,ultrasound,iontophoresis),
Dry needling (especially for Tendinitis),
Correctly practised stretching,strenghtening and stabilisation exercises,
Manipulation and mobiisation techniques,
Osteopathic approach,
Graston technique,
Local injection
In case no remedy occurs,surgery must be considered.

Frozen Shoulder

Frozen Shoulder (Adhesive Capsulitis) is stiffness,pain and limited range of motion in the shoulder joint.

CAUSES

Disuse because of pain;injury or a chronic health condition such as diabetes mellitus or a stroke,
Rheumatoid diseases,
Diminished synovial fluid in the joint,
After a surgery or injury,
Fibrosis within the joint that limits the movements

TREATMENT

Medical treatment to cease pain and swelling,
Heat or cold fomentations according to the stage of the disease,
Physical Therapy modalities,
ESWT(Exracorporeal Schock Wave Therapy),
Dry needling,
Cupping,
Manipulation and mobilisation,
Graston technique,
Osteopathic approach,
Correctly practised stretching and strenghtening exercises,
Intraarticular injections,
In case of failure and no progression is maintained in 4-6 months with conservative measures,surgery is recommended.

SHOULDER FRACTURES

The shoulder is a complex joint connecting the arm to the body.The shoulder bones include the humerus (upper arm bone),the scapula (shoulder blade) and the clavicle (collar bone).Disruption of any parts of the shoulder,fall on the shoulder,forced sports activity or a traffic accident may cause fracture of the bones.

TREATMENT

Depending on location,severity and the patient’s activity level; ‘figure 8 straps ,joint replacement or surgery; placing plates,screws or wires are done and the shoulder is immobilized in a proper position with a sling.

When the fracture is fully healed,Rehabilitation program must be started as soon as possible, for improving full range of motion and muscle strength.Physical therapy modalities and agreeable tissue mobilisation techniques are required.
